Make it easy on yourself and keep this very short. Someone mentioned 2 minutes, but that’s too long. You’re not really giving a speech – you’re giving a toast.
You could say something like you’re honored to be part of this wonderful day, partly because you love your mom and want to see her happy, but also because it means you get to officially welcome your stepdad into your family. You can tell an anecdote, like maybe something about the first time you met him, or you can stay serious and mention you’ve enjoyed watching their relationship get more serious and you can’t wait for what the future brings.
This can be easily done in a minute and that’s what I’d aim for.

The thing is, I'm a girl and her best friend. I have not been at a wedding in a while so I'm not sure how long a toast is supposed to go for and all that. What are some guidelines that should have?
Try to keep a minute or two. The long speeches are boring. If you are not usually a good public speaker, then now is the time to try to break some jokes or complicated stories. Keep it short sweet and simple. Try to avoid inside jokes. A quick make you laugh is fine if you can maliciously put it in the speech, but the guests will get confused if you go for five minutes on "the donkey in the basement and have no idea what that means. Keep clean! I once went to a wedding where the best man giving talked about walking in couples that sex on their first date, and you could hear a pin drop in the room, because everyone was so surprised and puzzled by them. There are no stories that would make grandma blush, no dirty words, etc. Just keep it sincere and honest. Something short and sincere is better than something long and made up. Talk about your relationship with him and try to work the bride somehow. Maybe something like, "I saw my older brother during years, and he was always there for me. So I wanted nothing more than to see you happy. And Sally, I can say that makes you happy because of the smile on his face whenever you're around. I am delighted to have you as a new sister. Salud! "

my family knows that I have fear of speaking in public but the extent of social anxiety (in their eyes) is a cop out. Aside from that I have been having hot flashes at work followed by chest pain, fear of walking and talking in front of even my boss or boyfriend. Maybe the doctor will give you a few tranquilizer pills that you might need once for that day.
